Chapter 5.16 TAXICAB DRIVERS-LICENSE AND REGULATIONS
5.16.040 License application.
Upon a blank form to be provided by the clerk-treasurer, each applicant
shall provide the following information:
(1) his full name and present
address;
(2) his place of residence for five years prior to the date of
application;
(3) his age, height, weight, color of eyes and
hair;
(4) his place of birth;
(5) whether he is a citizen of the United
States;
(6) whether he has been convicted of any felony or misdemeanor and,
if convicted, the nature of the offense or crime, and the date when and the
court where such conviction occurred;
(7) the fingerprints of the
applicant;
(8) whether the applicant previously has been licensed anywhere
as a taxicab driver, and if so, whether his license has ever been suspended and
when and for what cause;
(9) such other information as the board of trustees
may reasonably require.
The application shall be signed by the applicant
under oath and filed with the clerk-treasurer as a permanent record. (Ord. 382
